Registration Terms
After Allied Health professionals register, a copy of the attendees' Allied Health Certificate must be sent to to Gulfcoast Ultrasound Institute, Inc. at 4615 Gulf Blvd. Suite 205, St. Pete Beach, FL 33706 within 10 days of registration to receive proper credit, and for registration information to remain in our database.
Confirmation
Upon receipt of the registration payment, a confirmation letter will be sent to you along with arrival directions and class itinerary. For Florida programs, airline destination should be made to Tampa International Airport. Meeting location is approximately a 30-minute drive. Lodging arrival suggested evening prior to program start.
Tuition
All tuitions are subject to change without notice. Prepaid tuitions will be honored regardless of change status.
Deadlines
Registrations suggested at least 4 to 6 WEEKS PRIOR to program date. Since class enrollments are limited in size; early registration is advised. Registration is confirmed on a "first come, first serve" basis of PAID TUITIONS.
Advanced registration are accepted if registration payment is postmarked 21 days or more prior to the beginning date of the program. Any registration payments postmarked less than 21 days before the program MUST include an additional $40.00 late registration fee (when mailing in payment).
A full registration refund will be given if cancellation is received more than 30 days prior to a scheduled program. Registrations received 14-30 days prior to a program start date are non-refundable, and are limited to a one-time transfer restricted to the same person for the next identical program on the Gulfcoast Ultrasound Institute schedule. A $150.00 transfer fee plus any change in program tuition will apply. No refunds or transfers will be allowed less than 14 days to a program start date.

Course Discount
A 5% discount on course registration applies to individuals from the same facility that register for the same course and date at the same time and pay with the same form of payment (one combined payment) when attending courses in St. Pete Beach, FL. Not available when attending Combined courses. Cannot be combined with other course discounts/offers. Call us for details.
Cancellation
A full registration refund will be given if cancellation is received more than 30 days prior to a scheduled program/webinar. Registrations received 14-30 days prior to a program start date are non-refundable, and are limited to a one-time transfer restricted to the same person for the next identical program on the Gulfcoast Ultrasound Institute schedule. A $150.00 transfer fee plus any change in program tuition will apply. No refunds or transfers will be allowed less than 14 days to a program start date.
In the event a program is re-scheduled or cancelled, pre-paid attendees will be given the choice of transferring to another program or having the tuition refunded in full (if program is cancelled due to Acts of God, only transfers to another program will be offered). Gulfcoast Ultrasound will not be responsible for any travel and/or lodging reimbursements. Prices, schedules, and CME credits subject to change without notice.
International Orders
Unfortunately we are unable to ship orders outside of the United States prior to receiving payment. Checks from international clients must clear prior to shipping order. Call us at (727)363-4500 for bank wire transfer details.
ADA
In accordance with our policy to meet the requirements of The Americans with Disabilities Act (ADA), please notify the Program Coordinator at Gulfcoast Ultrasound Institute in writing at least two weeks prior to a scheduled program to request reasonable accommodations for a disability.
Tax Deduction An income tax deduction may be allowed for educational expenses (including registration fees, travel, meals and lodging) undertaken to maintain and improve professional skills (see Treasury Regulation 1.162-5 Coughlin vs. Commissioner 203F 3D307).
Accreditation
The Gulfcoast Ultrasound Institute is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
The Gulfcoast Ultrasound Institute designates this educational activity for a maximum of (*) AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity.
