Registration Terms
After Allied Health professionals register, a copy of the attendees' Allied Health Certificate must be sent to to Gulfcoast Ultrasound Institute, Inc. at PO Box 66708, St. Pete Beach, FL 33736 within 10 days of registration to receive proper credit, and for registration information to remain in our database.
Confirmation
Upon receipt of the registration payment, a confirmation letter will be sent to you along with arrival directions and class itinerary. For Florida programs, airline destination should be made to Tampa International Airport. Meeting location is approximately a 30-minute drive. Lodging arrival suggested evening prior to program start.
Tuition
All tuitions are subject to change without notice. Prepaid tuitions will be honored regardless of change status.
Deadlines
Registrations suggested at least 4 to 6 WEEKS PRIOR to program date. Since class enrollments are limited in size; early registration is advised. Registration is confirmed on a "first come, first serve" basis of PAID TUITIONS.
Advanced registration are accepted if registration payment is postmarked 21 days or more prior to the beginning date of the program. Any registration payments postmarked less than 21 days before the program MUST include an additional $40.00 late registration fee (when mailing in payment). HOSPITAL/GOVERNMENT PURCHASE ORDERS will be accepted from purchase order date.
Registrations received LESS than 30 days prior to a program start date are non-refundable and are limited to a one-time transfer for the next program/same person.
Please call if late payment or special arrangements need to be made.
Cancellation
A full registration refund will be given if cancellation is received four 4 weeks prior to a scheduled program. Cancellation after this time will result in a loss of the full tuition amount, unless the attendee transfers to the next available (same) program (one transfer limit).
In the event a program is re-scheduled or cancelled, pre-paid attendees will be given the choice of transferring to another program or having the tuition refunded in full (if program is cancelled due to Acts of God, only transfers to another program will be offered). Gulfcoast Ultrasound will not be responsible for any travel and/or lodging reimbursements. Prices, schedules, and CME credits subject to change without notice.
International Orders
Unfortunately we are unable to ship orders outside of the United States prior to receiving payment. Purchase orders from outside the United States are not valid. Checks from international clients must clear prior to shipping order. Call us at (727)363-4500 for bank wire transfer details.
ADA
In accordance with our policy to meet the requirements of The Americans with Disabilities Act (ADA), please notify the Program Coordinator at Gulfcoast Ultrasound Institute in writing at least two weeks prior to a scheduled program to request reasonable accommodations for a disability.
Tax Deduction An income tax deduction may be allowed for educational expenses (including registration fees, travel, meals and lodging) undertaken to maintain and improve professional skills (see Treasury Regulation 1.162-5 Coughlin vs. Commissioner 203F 3D307).
Accreditation
The Gulfcoast Ultrasound Institute is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to sponsor continuing medical education for physicians.
The Gulfcoast Ultrasound Institute designates the described educational activities for up to the indicated credit hours in Category 1 of the Physician's Recognition Award of the American Medical Association. Each physician should claim only those hours that he/she actually spent in the educational activity.
Note: While offering the CME credit hours noted above, activities are not intended to provide extensive training or certification for exam performance or interpretation.

SDMS: Society of Diagnostic Medical Sonographers
ARRT: American Registry of Radiologic Technologists (Cat. B credits only). ATTENTION: The ARRT will accept these activities as Category B CME credit until January 2008.

CME Quiz Terms
Gulfcoast Ultrasound Institute, Inc. will honor CME Quiz submissions for credit during the published/produced life of the enduring material, plus twelve months beyond the date of product discontinuation.
No CME credit can be awarded for quiz answers submitted after the twelve month discontinuation “grace period.”
Gulfcoast Ultrasound Institute, Inc. is not responsible for issuing discontinuation notices. Customers should refer to this website, current catalog, or call our administrative office to enquire about the CME status for any accredited enduring material that is utilized more that one year from the date of original purchase.
